Statement of Confidentiality

Information will not be released without your consent as required under the Health Insurance Portability and Accountability Act (HIPAA). Health information will only be disclosed to the person authorized to receive the information. Exceptions to this are as follows:

1. Subpoenaed by a court of law to testify in a matter.
2. Revelation that one has intent to harm oneself or someone else.
3. Reason to suspect that one will harm oneself or someone else, either by action or inaction.

If we have to testify in court, we will do all that we can to inform you of what we might have to reveal.
